PLEASE NOTE: The show operators must approve all food booths to be at the show. An application does not guarantee a spot.

Food Vendor Requirements: All food vendors are required to comply with the following regulations:

• Health Permit: A copy of your current Health Permit must be submitted as soon as possible to ensure Health Department approval. All food preparation and service must follow Health Department guidelines.
• Fire Safety: You must provide a fire extinguisher that meets Fire Department regulations.
• Electrical Needs: If power is needed, vendors must supply a 100 ft. industrial-grade extension cord and power strip.
• Sanitation: A three-compartment sink is required.
• Booth Use: Subleasing or sharing booth space is not permitted.
• Clean-Up: Vendors are responsible for cleaning their assigned booth area at the end of the event.
• Business License: A Special One-Day Glendora Business License must be purchased for $26.00 (visit the City of Glendora website for details).

I have read and understand the FLASHBACK application and agree to abide by all rules and regulations as stated. I acknowledge that:

There are no refunds or rain checks due to inclement weather or booth space assignment. I am required to display valid State Board of Equalization and Health Department permits at my booth. I may be removed from the event without a refund if I fail to comply with Health Department or Fire Department regulations, or if I deviate from the activities described in my application. I am responsible for providing and maintaining my own liability insurance. The Glendora Kiwanis Club, Glendora Chamber of Commerce, and the City of Glendora are not responsible or liable for any damages or losses incurred before, during, or after the event, including but not limited to loss or damage to work, equipment, or materials. No setup is allowed before 2:30 p.m. on the day of the event.

By participating in FLASHBACK, I agree to indemnify and hold harmless the Glendora Kiwanis Club, Glendora Chamber of Commerce, the City of Glendora, and their respective members, agents, employees, and volunteers from any liability, loss, claims, demands, costs, or judgments arising out of my participation or that of my guests, family, or representatives.
